To support a pilot arts-based intervention program for individuals with Alzheimer’s disease.

NJF Center for Caregivers, Inc.
$30,000 for one year
To develop an organizational sustainability plan.

Neighbors Who Care, Inc.
$40,345 for two years
To develop a caregiver support program.

Organization for Nonprofit Executives (ONE)
$10,000 for one year
For the creation of a Nonprofit Leadership Academy.

Cancer Workout Center
$18,000 for one year
For a pilot program to train personal trainers and exercise instructors to work with people living with cancer.

Rehoboth CDC
$50,000 for one year
For Canyon Corridor neighborhood community development.

Rebuilding Together Valley of the Sun, Inc.
$30,000 for one year
For a senior fall prevention services program.

Arizona Town Hall
$16,000 for one year
To fund the Arizona Health Dialogues project.

Arizona Facts of Life
$15,000 for one year
To engage community support services for participants in the Arizona Facts of Life program.

Arizona Health Facilities Authority
$150,000 for three years
To support local primary care planning in medically underserved areas by providing technical assistance and a small grant program.

AZ Lost Boys Center
$50,000 for one year
To support the development and further evolution of a culturally relevant health outreach program.

FIBCO Family Services

To support the development of the Village/Community Wells program.

Wesley Community Center
$35,000 for one year
To support a program to promote health safety in three west Phoenix neighborhoods.

Arizona Hands and Voices
$10,000 for one year
For a community outreach project for parents whose children are deaf or hard of hearing.

To conduct an assessment of the Hispanic/Latino community’s attitudes and beliefs with regard to advanced care directives.

$35,000 for one year
To develop a roadmap for universal vision screenings for Arizona’s children.
