Heart and Soul Initiative

Heart and Soul is a partnership between the African-American Faith Partnership and the Arizona Black Nurses Association to improve the health of their community by establishing a health promotion program in African American churches. The initial focus is to promote health awareness of cardiovascular and lung disease and corresponding changes in lifestyles of the congregations of participating churches.

The churches are predominately located in an urban setting in the Phoenix metropolitan area with an identifiable African-American population of 67,416. Participating churches include:
